EPIDURAL PRODECURES
Epidural steroid
injections are administered in pain clinics throughout the
world. Epidural anaesthesia is also a common treatment for pain in pregnant
women in delivery rooms. The overall goals include elimination
or reduction of pain. Though an invasive procedure itself,
another goal is to prevent need for more invasive treatments
including surgery.
Though a common
procedure in pain management clinics, questions arise as to
their safety and effectiveness. A review of studies indicates
that there is indeed controversy regarding them and disturbing
reports of serious neurological damage to otherwise healthy
individuals. Arachnoiditis is one of those adverse outcomes.
Patients, for the
most part, submit to epidural steriod injections trusting
that there will be no adverse reactions. Reports to ASAMS
support groups indicate that many persons developing arachnoiditis
as the result of an epidural procedure were not fully informed
of the risks. Due to its association with arachnoiditis, we
are presenting epidural related information.

The Anatomy of the Epidural Space consists of illustrated notes about
the components making up and associated with the epidural space. These
are used by students learning about regional anesthesia at the University of Washington in Seattle. It includes visual illustrations
and radiological images accompanying the written descriptions of lumbar,
thoracic and cervical epidural anatomy.

The National Health and Medical Research Council
The National Health and
Medical Research Council (NRMRC), Canberra, Australia completed a
report in 1994 entitled "Epidural use of steroids in the management
of back pain and sciatica of spinal origin." The report examines
the value of epidural use of steroids in the management of back pain
and is prepared primarily for use by health care professionals. The
link above presents the Executive summary and recommondations of the
NHMRC.
We gratefully thank The National Health and Medical Research Council
for granting permission to post this valuable information and to link
